Job summary

Advocate Medical Center in Winston-Salem is seeking a Case Manager to develop, implement, and evaluate case management outcomes for defined patient populations. You will collaborate with physicians, nurses, rehab, pharmacy, and other teams to optimize care and control costs.

The ideal candidate has five years of experience in patient care, RN licensure in the appropriate state, and is open to CM certification encouragement.

Qualifications

• Graduate of an accredited School of Registered Professional Nurses, BSN preferred, MSN encouraged.
• Minimum of five years of experience in the care of assigned patient population.
• Current RN licensure in the applicable state; Case Management certification encouraged.

Responsibilities

• Educate, consult, and assist in managing an identified patient population.
• Collaborate with physicians, nurses, rehabilitative therapies, pharmacy and other disciplines.
• Analyze patient care to improve outcomes, quality of life, and cost efficiency.
