Job summary

Southwest General is seeking a Care Manager to coordinate care across the episode, ensuring safe outcomes and efficient use of resources. You will oversee interdisciplinary activities, participate in bedside rounds, and partner with teams to shape discharge plans aligned with patient and family goals.

Ideal candidates have BSN (preferred) and at least 5 years of clinical nursing experience, plus current Ohio nursing licensure. Collaboration with post-discharge providers is essential.

Qualifications

• Minimum of five (5) years of recent clinical nursing or related nursing experience.
• BSN graduation preferred; accredited nursing school graduate.
• Current licensure by Ohio State Board of Nursing.

Responsibilities

• Carry out care coordination of the patient population to ensure safe outcomes across care continuum.
• Lead bedside interdisciplinary rounds and collaborate with the care team.
• Assess discharge needs and develop safe, effective discharge plans with input from care team.
• Coordinate referrals for continuing care and locate community resources for patients.
