Job summary

The Suites of Someren Glen is seeking a strong nurse leader to serve as Neighborhood/Unit Manager in our rehab and skilled nursing community. You will guide day-to-day nursing operations, coordinate care plans, and mentor CNAs, LPNs, and RNs to deliver high-quality resident care.

You will ensure PPS and MDS compliance, manage staffing, and collaborate with physicians, pharmacists, and families. This role requires clinical excellence, calm decision-making, and a commitment to compassionate,

Qualifications

• Active RN license in good standing in the state.
• Experience in rehab and skilled nursing required.
• Leadership or supervisory experience preferred.
• Knowledge of PPS and MDS processes.
• Excellent communication, coaching, and organizational skills.
• Ability to remain calm during clinical emergencies.
• Willingness to participate in on-call rotation.

Responsibilities

• Monitor residents' changing needs and coordinate updates with the care team.
• Perform pre-admission assessments to identify resident needs.
• Evaluate new tools and technology to enhance care.
• Serve as resource during emergencies and changes in resident condition.
• Maintain MDS compliance within the unit.
• Coordinate care plans and care conferences with interdisciplinary team.
• Oversee admission documentation.
• Lead frontline staff development with the Director of Nursing.
• Participate in on-call rotation.
• Other duties as assigned.
