North Houston Transitional Care in Houston, TX seeks an LPN/LVN to provide direct nursing care to residents and supervise nursing assistants, ensuring compliance with federal, state, and local standards.

You will manage day-to-day nursing operations, complete charting, administer medications as ordered, participate in care plans, and collaborate with the Nurse Supervisor to maintain quality resident care.

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Must possess a current, unencumbered, active LPN/LVN license in the state.
• Must be able to read, write, and understand English.
• Experience in long-term care setting preferred.

Responsabilités

• Inform nursing personnel of new admissions and room assignments.
• Greet residents upon admission and escort to rooms.
• Provide direct nursing care under supervision of RN/Director of Nursing.
• Participate in care planning and discharge planning.
• Complete and file resident medical records and documentation.
