Job summary

Houstonidence Opco, LLC is seeking an experienced LPN/LVN nurse supervisor to lead the nursing team in a long-term care setting. The role focuses on directing nursing assistants, maintaining care quality, and ensuring adherence to federal, state, and local standards.

The position requires active licensure, strong leadership, and the ability to develop and monitor care plans, documentation, and QA activities.

Qualifications

• Must possess a Nursing Degree or be a graduate of an approved LPN/LVN program.
• Must hold a current, unencumbered license to practice as an LPN/LVN in the state.

Responsibilities

• Direct the day-to-day functions of nursing assistants and ensure compliance with policies.
• Review and revise nursing service procedures, manuals, and discharge plans as required.
• Coordinate staffing, plan shifts, and oversee charting, documentation and reporting.
• Administer medications as ordered and verify resident identity before treatment.
• Maintain quality assurance processes and participate in facility surveys and safety plans.
• Provide leadership, supervise staff performance, and ensure adherence to nursing standards.
