Job summary

Oak Street Health is seeking a Nurse Practitioner to provide effective, equitable value-based primary care to adults on Medicare in outpatient clinics. You will work within a collaborative care team to build meaningful patient relationships and support care plans.

The role includes office visits, wellness visits, health risk assessments, and care coordination with physicians, specialists, and care managers. Expect 40 hours per week in a full-time position with competitive benefits.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program.
• National certification in at least one of these specialties: Family NP, Adult-Gerontology Primary Care NP, Adult NP, Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer wellness visits and health risk assessments with thorough documentation.
• Coordinate care with other providers, specialists, testing facilities, and agencies.
• Lead population health initiatives with the care team.
