gobravvo in Lancaster, PA is seeking a Nurse Practitioner (NP) to deliver value-based primary care to Medicare patients. You will work with an interdisciplinary team to provide patient-centered care, manage chronic and acute conditions, and participate in quality initiatives.

The role emphasizes preventive medicine, collaborative care, and access to on-site diagnostic resources. This full-time position offers a base salary with performance incentives, strong benefits including CME support,

Qualifications

• MSN, DNP, or NP certificate from an accredited program.
• National certification as FNP/AGPCNP/Adult NP/Gerontological NP.
• Active, unrestricted NP license and active DEA license.
• Experience with HCC, ICD-10 coding, and health risk assessments preferred.
• Bilingual Spanish-speaking skills are highly valued.

Responsibilities

• Provide comprehensive primary care to adult and geriatric patients.
• Conduct 20-minute and 40-minute visits for regular and new patients.
• Maintain patient visit volumes of 13–16 per day.
• Document care in EMR systems (Canopy and Greenway).
• Collaborate with scribes, medical assistants, and social workers.
• Coordinate care transitions and chronic disease management.
• Engage patients through community activities and transportation/wellness programs.
• Participate in quality improvement initiatives and organizational goals.
