gobravvo in Columbia, SC seeks a full-time Nurse Practitioner (NP) to join a value-based primary care team serving Medicare populations. You will collaborate with physicians, scribes, MAs, and social workers, with on-site labs and advanced telehealth to support outcomes.

Qualifications

• Master of Science in Nursing and NP certification with RN licensure.
• National NP certification in a relevant family/primary care track.
• Active, non-probationary state NP license and DEA license.

Responsibilities

• Provide primary and preventive care to Medicare patients as an NP.
• Manage a patient panel with average 13-16 visits per day.
• Conduct 20-minute visits and 40-minute assessments for complex patients.
• Collaborate with physicians, scribes, medical assistants, and social workers.
• Utilize on-site labs and telehealth access to specialists.
• Document care using Canopy and Greenway EMR platforms.
• Support value-based care initiatives to reduce hospital admissions.
• Engage in community activities and patient wellness programs.
• Ensure regulatory compliance in documentation (HCC, ICD-10, HRA).
• Support patient transportation within a 5-mile radius.
