Job summary

gobravvo is seeking a dedicated Nurse Practitioner to join a value-based care team serving Medicare populations. You will provide comprehensive primary care with a focus on quality outcomes, collaborating with physicians, scribes, and social workers to support holistic patient wellness.

You will work in a collaborative, multidisciplinary setting, using EMR systems Canopy and Greenway, and participate in community programs while maintaining manageable patient volumes to ensure thorough care and

Qualifications

• Graduate nursing degree (MSN or DNP) or an NP certificate.
• Active state NP license and DEA registration.
• Knowledge of ICD-10 coding and HRAs preferred.
• Experience in value-based care is a plus.

Responsibilities

• Provide comprehensive primary care to Medicare patients.
• Perform patient visits including diagnostic and new-patient assessments.
• Collaborate with physicians, staff, and social workers in a multidisciplinary team.
• Utilize EMR systems (Canopy, Greenway) for documentation and patient management.
