Job summary

Gobravvo seeks a dedicated Nurse Practitioner (NP) to deliver value-based care to Medicare populations in a Chicago clinic. The role emphasizes preventive medicine and chronic disease management within a multidisciplinary team, supported by on-site labs and modern EMR systems.

The NP will manage a Medicare patient panel with regular visits, coordinate with physicians, scribes, social workers, and phlebotomy staff, and contribute to community wellness initiatives.

Qualifications

• Master of Science in Nursing or higher and NP certification.
• Active, non-probationary NP license and DEA license.
• Experience with HCC documentation, ICD-10 coding, and HRAs preferred.

Responsibilities

• Manage a panel of Medicare patients, averaging 13-16 visits daily.
• Conduct regular patient visits and comprehensive diagnostic assessments.
• Collaborate with scribes, medical assistants, social workers, and physicians.
• Utilize on-site laboratory and specialist access for comprehensive care.
• Ensure accurate documentation using EMR systems Canopy and Greenway.
• Engage patients in community wellness activities and promote preventive care.
