Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking an autonomous Nurse Practitioner or Physician Assistant to provide primary and episodic care in a skilled nursing facility. You’ll manage your own panel and cultivate lasting relationships with patients and families.

Routines feature 6-hour shifts with morning rounds, no on-call, charting from home, and productivity-based pay. Comprehensive benefits, CME stipend, and modern tech support are included.

Qualifications

• Board certification required (e.g., AGNP, FNP; PA-C) — active and unrestricted.
• Valid, unrestricted NP or PA license in the state of operation.
• Master's degree in Nursing (NP) or PA program required for credential.
• Long-term care or skilled nursing experience strongly preferred.
• Strong independent clinical judgment and ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients per day, mix of chronic care and acute issues.
• Perform annual wellness visits and H&Ps; manage chronic conditions.
• Coordinate with collaborating physicians, specialists, and facility staff.
• Communicate with families and participate in care conferences.

Nurse Practitioner or Physician Assistant — Skilled Nursing Facility

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–20 patients a day — a mix of long-term chronic care and acute issues.
