Job summary

United Family Medical Center is seeking a compassionate Nurse Practitioner to provide care to residents in skilled nursing facilities in the Amesbury and Haverhill area, with primary responsibilities in Danvers and surrounding communities.

In this role you will conduct patient assessments, develop treatment plans, manage acute and chronic conditions, and collaborate with physicians, nurses, rehabilitation staff, and other care team members to ensure high-quality, patient-centered care.

Qualifications

• Active NP license in Massachusetts.
• Current board certification (FNP, ANP, or relevant specialty).
• DEA license and prescriptive authority in Massachusetts.
• Experience in primary care, family medicine, or a related clinical setting preferred.
• Strong clinical assessment, critical thinking, and diagnostic skills.
• Excellent communication skills and a compassionate, patient-first approach.

Responsibilities

• Conduct patient assessments, develop treatment plans, and manage acute and chronic conditions.
• Provide ongoing care, follow-up visits, and medication management.
• Collaborate with physicians, nurses, rehabilitation staff, and other care team members.
• Support transitions of care and communicate with patients and families.
• Maintain accurate and timely clinical documentation.
