Job summary

Conviva Senior Primary Care is seeking a Nurse Practitioner or Physician Assistant for a float role in the South San Antonio clinics. You will work onsite within a multidisciplinary team delivering personalized, high-quality care to seniors and coordinating referrals and services.

The role reports to the Associate Medical Director and emphasizes value-based care with a focus on comprehensive geriatric management, chronic condition care, and team collaboration.

Qualifications

• Master's degree in Nursing or PA program completion with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• TB screening may be required for selection.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team for coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Demonstrate a passion for value-based care.
