Job summary

Conviva Senior Primary Care in Jourdanton, TX is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join a collaborative, multidisciplinary team delivering personalized, high-quality care to seniors onsite at our Jourdanton clinic.

You will work 40 hours per week, report to the Associate Medical Director, participate in on-call rotation, manage referrals, hospital/SNF coordination, DME and home health services, and document care with support from quality-based coders, all to

Qualifications

• Master's Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• TB screening required if selected.
• 2+ years of value-based care or Medicare/Medicare Advantage experience preferred.
• Active DEA license and Medicare/Medicaid numbers preferred.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately with support from quality-based coders.
• Participate in on-call rotation and support center growth.
• Deliver value-based care to patients.
