Job summary

Humana's Conviva Senior Primary Care clinic in Jourdanton, TX seeks a Nurse Practitioner or Physician Assistant to join a multidisciplinary team delivering value-based care to seniors. You will work onsite, report to the Associate Medical Director, and participate in on-call rotations to support growth at the center.

Ideal candidates hold an active NP/PA license, a master's degree in nursing or PA certification, and have a focus on high-quality, compassionate senior care.

Qualifications

• Master's Degree in Nursing or PA program with board certification.
• Active NP/PA license in the state of practice.
• TB screening may be required.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with your care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately and efficiently with support from quality-based coders.
• Participate in on-call rotation and support growth plans at your center.
• Demonstrate passion for value-based care.
