Job summary
CenterWell in Jacksonville, FL is seeking a full-time Nurse Practitioner or Physician Assistant to join our Conviva Senior Primary Care team. You will provide comprehensive, value-based primary care to seniors, collaborating with a multidisciplinary team to coordinate referrals, hospital/SNF work, DME and home health services.
The role requires an active Florida NP/PA license, a master’s degree in nursing, DEA license, and experience in Medicare/Medicaid settings.
Qualifications
- Master’s degree in nursing with board certification.
- Active, unrestricted NP, or PA license in Florida.
- Experience in value-based care, primary care or Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
- Familiarity with HEDIS quality indicators and Medicare guidelines.
Responsibilities
- Provide comprehensive care to seniors following clinical standards.
- Collaborate daily with your care team to ensure coordinated, high-quality care.
- Manage referrals, hospital/SNF coordination, DME, and home health services.
- Document care with support from quality-based coders.
- Participate in on-call rotation and support growth plans at your center.
- Have experience with providing value-based care.
- Work on-site.
