Job summary

CenterWell Senior Primary Care is seeking a Nurse Practitioner or Physician Assistant to join our multidisciplinary team, delivering comprehensive, value-based care to seniors. You’ll collaborate with physicians, pharmacists, care coaches, and behavioral health specialists to provide personalized, high-quality care at our centers.

Qualifications

• Master’s Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Experience in value-based care or Medicare Advantage preferred.

Responsibilities

• Provide comprehensive care to seniors in accordance with clinical standards.
• Collaborate daily with care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Participate in daily huddles and on-call rotation.
• Ensure compliance with licensing and accreditation requirements.
