Job summary

Humana Conviva Senior Primary Care in Jacksonville, FL is seeking a full-time Primary Care Nurse Practitioner or Physician Assistant to join our clinic team. You will provide comprehensive senior care, collaborate with your care team, and coordinate referrals, hospital/SNF and home health services on-site.

The role emphasizes value-based care, documentation support from quality coders, and participation in on-call rotations to support growth at the center.

Qualifications

• Master's degree in nursing is required and board certified.
• Active, unrestricted NP or PA license in Florida.
• DEA license and Medicare/Medicaid numbers or ability to obtain them.
• Familiarity with HEDIS indicators and Medicare guidelines.

Your Role & Responsibilities

• You will provide comprehensive care to seniors following clinical standards.

What We're Looking For

Required:

• Master's degree in nursing with board certification.
• Active, unrestricted NP, or PA license in the state of Florida.
• Experience in value-based care, primary care or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
• Familiarity with HEDIS quality indicators and Medicare guidelines.

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
