Humana Inc. is seeking a full-time Primary Care NP or PA to join Conviva Senior Primary Care in Jacksonville, FL. You’ll be part of a collaborative team delivering personalized, high-quality care to seniors, with on-site responsibilities and a focus on value-based outcomes.

You will report to the Associate Medical Director and work in a clinic that emphasizes team-based care, longer patient visits, and a supportive culture focused on growth and well-being.

Qualifications

• Master’s degree in nursing with board certification.
• Active, unrestricted NP or PA license in Florida.
• Experience in value-based care, primary care or Medicare Advantage.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain them.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team for coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support center growth plans.
• Work on-site and provide value-based care delivery.

As a Nurse Practitioner (NP) or a Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
