Conviva Senior Primary Care in Jourdanton, TX, is hiring a Nurse Practitioner or Physician Assistant to join a collaborative, multidisciplinary team onsite. You will deliver comprehensive care to seniors and coordinate with physicians, pharmacists and care coaches.

The role offers a competitive base salary with a pay range of $123,800–$170,300 per year, eligibility for a bonus plan, and a benefits package including medical, dental, vision, 401(k), time off, disability and life insurance.

Qualifications

• Master's in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• TB screening if selected for the role.
• Active DEA license and Medicare/Medicaid numbers preferred.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.

As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. In this role, you will work onsite at our Jourdanton clinic. You will report to our Associate Medical Director.

What We're Looking For

• Required: You will have a Master's Degree in Nursing or completion of a PA program with board certification.
• Required: You will have an active, unrestricted NP or PA license in the state of practice.
• Required: If selected for this role, you will be required to be screened for TB.
• Preferred: You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings.
• Preferred: You will have an active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
