Job summary

Gary and Mary West PACE in California seeks a Nurse Practitioner to provide primary care and ongoing medical coverage for PACE participants. You will work within an interdisciplinary team to develop comprehensive care plans, coordinate with physicians, and support home- and center-based care.

The role emphasizes geriatrics, chronic disease management, and collaborative decision-making, with a focus on maintaining independence and quality of life for seniors.

Qualifications

• Graduate of an accredited school of nursing.
• RN State License and NP certification with prescriptive authority.
• DEA license and BLS/CPR training are required.
• Current driver's license and auto insurance are required.

Responsibilities

• Provide primary care and acute illness evaluation and treatment in collaboration with PACE Physicians.
• Perform comprehensive assessments and coordinate with the IDT to develop care plans.
• Collaborate on discharge planning from acute or long-term care settings.
• Refer participants to medical specialists as indicated; manage care in nursing homes.
• Participate in rotating night and weekend calls and ensure care coordination.
