Gary and Mary West PACE seeks a Nurse Practitioner to provide primary and acute care for frail seniors within a multidisciplinary team in the North County San Diego area. The role includes comprehensive assessments, care planning, and coordination with PACE physicians.

The position requires NP certification with prescriptive authority, DEA, and active RN license. Strong collaboration, communication, and commitment to geriatric care are essential.

Formación

• RN license required.
• Nurse Practitioner with prescriptive authority required.
• DEA registration required.
• Current BLS/CPR certification.
• Valid driver's license and auto insurance.

Responsabilidades

• Provide primary and acute care for participants both in-center and in-home.
• Perform initial and periodic comprehensive assessments with IDT coordination.
• Collaborate with physicians and IDT on care plans and discharge planning.
• Refer participants to medical specialists as indicated.
• Manage care in nursing home settings with regular visits.
• Actively participate in IDT and 24-hour care coordination.
• Participate in rotating night and weekend calls.
• Review outcomes/utilization data to improve care and policy.
• Protect privacy and maintain confidentiality of all information.
• Follow GMWP policies and participate in required QA activities.
• Maintain licensure and certifications.
