Job summary

Career Land Center, LLC is seeking a Nurse Practitioner to join an interdisciplinary care team at a PACE center in Stockton, CA. You will provide primary care to frail elderly participants in a center-based setting, collaborating with a supervising physician and IDT members.

Requirements include an NP degree, CA license, and national certification; DEA registration is needed. Experience with geriatric populations and prior PACE exposure are preferred.

Qualifications

  • Master's or Doctoral degree in Nursing from an accredited NP program.
  • Active, unrestricted California NP license, or ability to obtain prior to start.
  • National certification (ANCC or AANP) in Family, Adult-Gerontology, or Geriatric practice preferred.
  • Current DEA registration, or ability to obtain.
  • Experience with geriatric or medically complex populations preferred.
  • Prior experience in a PACE program or similar interdisciplinary care model preferred.

  • Provide primary care services to a panel of PACE participants, most of whom are frail, elderly with complex conditions, in collaboration with the supervising physician.
  • Conduct comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop treatment plans.
  • Be an active member of the IDT, collaborating with nursing, social work, rehabilitation, dietary, recreation, and personal care staff.
  • Participate in regular IDT meetings.
  • Coordinate participant care across settings, including center visits, home care coordination, and referrals.
  • Order and interpret diagnostic tests, prescribe and manage medications within scope and collaborative agreement.
  • Maintain accurate documentation in the EMR.
  • Support quality improvement initiatives and CMS/state PACE requirements.
  • Participate in an on-call rotation as needed.

Job description

A PACE (Program of All-Inclusive Care for the Elderly) center opening in the Stockton, CA area is seeking a Nurse Practitioner to join its interdisciplinary care team. PACE is a capitated care model funded through Medicare and Medicaid that supports frail, elderly participants living safely in their homes and communities through comprehensive, coordinated care delivered in a center-based, outpatient clinic setting. This is not a home visit or home care position; participants are seen at the PACE center.

Responsibilities
  • Provide primary care services to a panel of PACE participants, most of whom are frail, elderly individuals with complex, multiple chronic conditions, in collaboration with the center's supervising physician.
  • Conduct comprehensive health assessments, diagnose and manage acute and chronic conditions within scope of practice, and develop individualized treatment plans.
  • Serve as an active member of the interdisciplinary care team (IDT), collaborating with nursing, social work, rehabilitation therapy, dietary, recreation, and personal care staff.
  • Participate in regular IDT meetings.
  • Coordinate participant care across settings, including center-based visits, home care coordination, and specialist or hospital referrals.
  • Order and interpret diagnostic testing, and prescribe and manage medications within scope of practice and collaborative agreement.
  • Maintain accurate, timely documentation in the electronic medical record.
  • Support quality improvement initiatives and compliance with CMS and state PACE program requirements.
  • Participate in an on-call rotation as needed.
