Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to oversee a panel of nursing home residents. You will perform comprehensive assessments, craft individualized care plans and collaborate with a clinical leadership team to optimize member outcomes.

You will engage families, coordinate with PCPs and the care team, and ensure care aligns with members’ goals. Strong NP/PA credentials and nursing home experience are preferred, with autonomous practice in a team-driven setting.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MSN as a Nurse Practitioner from a nationally accredited program.
• Experience in nursing home environment preferred.
• Strong communication with physicians, members and families.
• Self-directed yet collaborative in a team setting.
• Excellent clinical skills: assessment, diagnosis, treatment and prescribing.

Responsibilities

• Provide collaborative primary care to a panel of long-term care members.
• Lead ISNP Model of Care activities and advanced care planning.
• Coordinate with health plan operations: prior authorizations, services, quality gaps.
• Serve as advisor to members, families, and facility staff.
• Maintain frequent communication with PCPs and interdisciplinary team.
• Contribute to market goals while managing member caseloads.
