You will work with physicians, care teams, and families to manage acute and chronic conditions, build strong patient relationships, and enhance outcomes using our innovative care model and advanced technology. This role emphasizes autonomy within a mission-driven organization and travel to assigned facilities.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Team-oriented with punctuality and organizational skills.

Responsibilities

• Deliver high-quality primary care through comprehensive assessments and care plans.
• Diagnose, treat, and manage acute and chronic conditions with preventive focus.
• Provide ongoing care management to improve quality of life.
• Build meaningful patient relationships through regular facility rounds.
• Coordinate with physicians, staff, and families to plan care across the continuum.
• Maintain EMR documentation and ensure regulatory compliance.
