Job summary

Brighton/Suncrest Hospice is seeking a Nurse Practitioner to perform physical examinations and preventive health measures within prescribed guidelines. The NP will record findings, develop care plans, and coordinate with physicians and other health professionals to ensure comprehensive patient care.

The ideal candidate will be licensed as a Nurse Practitioner, be able to drive with reliable transportation, and have a commitment to collaborative hospice care.

Qualifications

• Requires completion of accredited Nurse Practitioner program.
• State licensure in good standing.
• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance.
• Regular lifting and transferring patients as required.

Responsibilities

• Records physical findings, and formulates plan and prognosis, based on patient's condition.
• Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
• Prescribes or recommends drugs or other forms of treatment.
• Monitors and maintains progress of patients and reports progress to supervising physician.
• Has the ability to work collaboratively with hospice employees and volunteers as part of an ID.
