Job summary

CenterWell Senior Primary Care in Louisiana is seeking a dedicated Nurse Practitioner for a PRN (as-needed) role at our Shreveport/Bossier City clinics. This zero-hour contract supports flexible scheduling while delivering high-quality outpatient care to seniors.

You will provide comprehensive senior care, collaborate with the care team, and maintain accurate medical histories. A Master's Degree in Nursing with board certification and an active NP license are required, along with a DEA license

Qualifications

• Active, unrestricted NP license in the state of practice.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and Medicaid and Medicare numbers.
• TB screening may be required.

We are looking for a dedicated and experienced Primary Care Nurse Practitioner to join our team on a PRN (as-needed) basis. This is a zero hour contracted role, ideal for providers looking for flexible scheduling while contributing to high-quality outpatient care.

You will manage a variety of health-related concerns, focusing on continuity of care, health maintenance, and disease prevention.

You will work onsite at our clinics in Shreveport and Bossier City reporting to our Associate Medical Director.

Your Role & Responsibilities

• You will collaborate daily with your care team to ensure coordinated, high-quality care.


• You will maintain accurate and up-to-date medical histories and records.

What We're Looking For

• You will have a Master's Degree in Nursing with board certification.


• You’ll hold an active, unrestricted NP license in the state of practice.


• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and Medicaid and Medicare numbers.


• If selected for this role, we will require you to undergo TB screening.
