Job summary

CenterWell Senior Primary Care in Florida is seeking an advanced practice provider (NP/PA) to deliver comprehensive care to seniors within a team-based model. The role emphasizes coordinated, high-quality care with referrals management and compliance with licensing requirements.

The position is part-time (Scheduled Weekly Hours: 20) and includes benefits such as PTO, CME time, and participation in a bonus incentive plan.

Qualifications

• Master’s degree in nursing or PA program completion.
• Board certification is required.
• Active, unrestricted NP or PA license in the practice state.
• At least one year of primary care experience.
• Experience with electronic medical records (EMR).

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Participate in daily huddles and document care accurately with support from coders.
• Ensure compliance with licensing and accreditation requirements.

Required Qualifications

Your Role & Responsibilities

What We’re Looking For

Required:

• Master’s Degree in nursing or completion of a physician assistant program
• Board certification
• Active, unrestricted NP or PA license in the state of practice.
• One year of experience in primary care
• Experience with electronic medical records

If selected for this role, you will be required to be screened for TB.

Preferred Qualifications

• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
