Job summary

Humana CenterWell Senior Primary Care seeks a Nurse Practitioner or Physician Assistant to deliver comprehensive, value-based care for seniors at our State Ave. clinic. You will work with physicians, pharmacists and care teams to coordinate referrals, hospital coordination, DME and home health services.

You will document care with support from coders, participate in on-call rotations, and contribute to center growth plans while upholding high-quality standards.

Qualifications

• Master's Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• TB screening required.
• Spanish speaking ability.

Your Role & Responsibilities

What We're Looking For

Required:

• You will have a Master's Degree in Nursing or completion of a PA program with board certification.
• You will have an active, unrestricted NP or PA license in the state of practice.
• If selected for this role, we will require you to undergo TB screening.
• Spanish speaking

Preferred:

• You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings.
• You will have an active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and Medicaid and Medicare numbers

Additional Information:

Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. The Federal Government will require applicants to take the Interagency Language Rating (ILR) test.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
