Humana Inc. through CenterWell Senior Primary Care is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our senior primary care team in an outpatient clinic setting.

You will work within a collaborative care model alongside physicians, pharmacists, and care coaches to deliver value-based care to seniors. The role emphasizes patient-centered care, experience with referrals and care coordination, and participation in on-call rotations.

Qualifications

• Bachelor's degree in a relevant field.
• 8 or more years of technical experience.
• 2 or more years of project leadership experience.
• Active, unrestricted Nurse Practitioner/PA license in the appropriate state.

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with your care team to ensure coordinated, high-quality care.

Your Role & Responsibilities

This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. Must be passionate about contributing to an organization focused on improving consumer experiences. Must be willing to participate in limited on-call coverage by phone #LI-CB1. Scheduled Weekly Hours 40.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $111,400 - $153,300 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
