Job summary

Humana Inc. through Conviva Senior Primary Care in Lake Charles, LA, is hiring a Nurse Practitioner or Physician Assistant to provide comprehensive geriatric care in a collaborative, team-based setting.

You will support referrals, hospital coordination, DME and home health services while practicing value-based care, reporting to the Associate Medical Director. Expected hours are full-time with competitive compensation and benefits.

Qualifications

• Master’s Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in state of practice.
• TB screen may be required before start.
• Report to the Associate Medical Director.

Your Role & Responsibilities

What We’re Looking For

• You will have a Master’s Degree in Nursing or completion of a PA program with board certification.
• You will have an active, unrestricted NP or PA license in the state of practice.
• If selected for this role, we will require you to screen for TB.
• You’ll report to the Associate Medical Director.

Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
