Job summary

Atrius Health, part of the Optum family, is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to join our Boston, MA team. You will provide comprehensive clinical care, coordinate care for a patient panel, and work within a physician-led network focused on value-based care.

You will monitor and manage acute and chronic illnesses, develop care plans, and collaborate with patients, families and staff to deliver high-quality, coordinated care across locations.

Qualifications

• Must be a graduate of an accredited NP/PA program.
• Must hold an active, unrestricted MA NP/PA license.
• National Board Certification required.
• BLS certification required; DEA registration or ability to obtain post-hire.
• Mass DPH CS registration or ability to obtain post-hire.

Responsibilities

• Coordinate clinical and administrative care for a patient panel.
• Perform examinations, diagnoses, and treatments in primary care; manage care plans with referrals.
• Oversee monitoring of acute and chronic illnesses; supervise care plans.
• Collaborate with patients and families to manage care plans.
• Provide consultation for hospitalized patients within the panel.
• Provide direction and support to staff to ensure department effectiveness.
