Job summary

Atrius Health, part of Optum, is seeking a Nurse Practitioner to join our Dedham, MA team. You will provide comprehensive, longitudinal clinical and administrative coordination for a defined patient panel, performing exams, diagnoses and treatments in a primary care setting with care plan supervision and referrals as needed.

You will collaborate with patients and families, oversee the monitoring of acute and chronic illnesses, and support staff to ensure effective delivery of care.

Qualifications

• Graduate of an Accredited Nurse Practitioner Program, Masters level preferred.
• Licensed by the Commonwealth of Massachusetts board of registration in Nursing.
• National Board Certification.
• Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
• Current active Mass DPH Controlled Substances Registration or ability to obtain post-hire, per state regulations.
• Maintains BLS certification, professional certification as appropriate.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of all care for a specific panel of patients.
• Performs examinations, diagnoses and treatment in a primary care setting, including supervision of care plans and referrals.
• Oversees monitoring and management of patients with acute and chronic illnesses and documents care plans.
• Partners with patients and families to manage care plans and provides staff direction to assure departmental effectiveness.
