Qualifikationen

• Graduate of an Accredited Nurse Practitioner or Physician Assistant Program.
• Active, unrestricted Massachusetts NP or PA license.
• National Board Certification.
• Current active Mass DPH CS Registration or ability to obtain post-hire.
• Current active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Maintains BLS certification, professional certification as appropriate.

Aufgaben

• Comprehensive, longitudinal clinical and administrative coordination of care for a panel of patients.
• Examination, diagnosis and treatment in a primary care setting with care plan supervision and referrals as needed.
• Monitor and manage patients with acute and chronic illnesses; serve as care plan author and supervisor.
• Partner with patients and families to manage care plans.
• Provide consultation regarding hospitalized patients in the panel.
• Provide direction and support to staff to ensure departmental effectiveness.
