Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program.
• Active, non-probationary state NP license.
• Active DEA license.
• U.S. work authorization.

Responsibilities

• Provide effective primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments.
• Coordinate care with physicians, specialists, labs and care managers.
• Lead population health initiatives within the care team.
