Job summary

Oak Street Health in Indianapolis seeks a Nurse Practitioner to deliver value-based primary care to adult patients on Medicare. You will collaborate with physicians and care teams, conduct visits, manage annual wellness visits, and lead population health initiatives within an outpatient setting.

Requirements include a MSN/ DNP or NP cert, active NP license in Indiana, DEA license, US work authorization, and relevant primary care experience.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in one of: Family NP, Adult-Gerontology Primary Care NP, Adult NP, Gerontological NP.
• Active state NP license (Indiana)
• Active DEA license
• US work authorization

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the care team.
• Assisting the care team with phone triage and outreach.
• Educating patients on their health conditions and care plans.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties as assigned.
