Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate completed.
• National NP certification in one of the listed specialties.
• Active, non-probationary NP license in the state.
• Active DEA license required for practice.
• Authorized to work in the United States.

Responsibilities

• Provide effective, value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Deliver longitudinal care and build patient relationships for trust.
• Work on a collaborative care team in outpatient settings.
• Coordinate care with specialists, testing facilities and care managers.
• Engage in population health initiatives and preventive care tasks.
• Participate in promotional and outreach activities as needed.
• Conduct home visits when required and appropriate.
