Oak Street Health in Cleveland, OH seeks a Nurse Practitioner to deliver value-based primary care to adults on Medicare. You will work with a collaborative care team on outpatient clinics, providing longitudinal care, building trust, and guiding patients through their care plans.

Qualifications

MSN, DNP, or NP certificate program graduate.
National certification in Family NP, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
Active, non-probationary state NP license.
Active DEA license.
US work authorization.

Responsibilities

Provision of exceptional primary care.
Conduct office visits for routine and acute issues.
Administer Annual wellness visits and health risk assessments, with holistic documentation.
Care coordination with providers, specialists, testing facilities, and agencies.
Population health leadership in coordination with the Care Team.
Assisting the care team with phone triage and outreach.
Educating patients on their health conditions, care plans, and treatments.
Participating in Oak Street Health promotional activities.
Conducting home visits as needed.
Other duties, as assigned.
