Oak Street Health in Cleveland, OH is seeking a Nurse Practitioner to provide value-based primary care to adults on Medicare in outpatient settings. You will work with a collaborative care team to deliver longitudinal care, build relationships, and support care plans.

The role emphasizes population health leadership, care coordination, and patient education. Requires NP training and state license; competitive pay and comprehensive benefits; 40-hour weeks.

Qualifications

• Master of Science in Nursing or higher degree required.
• National certification in Family NP, Adult-Gerontology NP, Adult NP or Gerontological NP.
• Active, non-probationary NP license in the United States.
• Active DEA license and US work authorization.

Responsabilités

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer wellness visits and health risk assessments.
• Coordinate care with providers, specialists, testing facilities, and agencies.
• Lead population health initiatives with the care team.
• Assist with triage and patient outreach.
• Educate patients on health conditions and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
