Oak Street Health is seeking a Nurse Practitioner to deliver value-based primary care for adults on Medicare in our Dayton outpatient clinic. You will work on a collaborative team including physicians, PAs, medical assistants, a scribe, and care managers to provide longitudinal care and build trust with patients.

Responsibilities include conducting visits, wellness checks, health risk assessments, and coordinating with specialists and facilities.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• Active, non-probationary state NP license.
• Active DEA license.
• US work authorization.

Responsabilités

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
