Job summary

Oak Street Health is seeking a Nurse Practitioner to provide effective primary care to adults on Medicare within a value-based care model. You will work on a collaborative care team, conducting visits, wellness assessments, and care coordination to keep patients healthy and out of the hospital.

The role emphasizes longitudinal patient relationships, documentation, and participation in promotional activities, with full-time hours and a 40-hour work week.

Qualifications

• MSN or DNP with NP certification.
• National NP certifications: FNP, AGPCNP, ANP, GNP.
• Active NP license in the state.
• Active DEA license.
• US work authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and HRAs with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.

Education

Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)

Nurse Practitioner Certification (NP)

National certification in Family Nurse Practitioner; Adult-Gerontology Primary Care NP; Adult NP; Gerontological NP

Active state NP license

Active DEA license

US work authorization
