Oak Street Health in Evanston, IL seeks a Nurse Practitioner to deliver value-based primary care to adults on Medicare. The NP works with physicians and a care team to provide longitudinal care, wellness visits, and disease management, focusing on patient relationships and outcomes.

The role involves collaboration with a physician, nurse practitioner or PA, MAs, a scribe, and care managers in outpatient clinics. It requires an NP license, DEA, and US work authorization, with 40 hours weekly.

Qualifications

• Must hold MS in nursing, DNP, or NP certificate.
• Active, non-probationary NP license in the state.
• Active DEA license.
• US work authorization.
• Preferred: experience in primary care, geriatrics, or family medicine.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and HRAs with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the care team.
• Assist the care team with phone triage and outreach.
• Educate patients on health conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties, as assigned.
