- Administer annual wellness visits and health risk assessments with thorough documentation.
- Care coordination with other providers, facilities, and agencies.
- Lead population health initiatives with the care team.
- Educate patients on health plans and treatments.
- Assist with phone triage and outreach.
- Participate in Oak Street Health promotional activities.
- Conduct home visits as needed.
