Job summary
Upperline Health in Augusta, GA is seeking a Nurse Practitioner to provide clinic-based care and coordinate care across home visits and telehealth. You will develop plans of care for adults with chronic conditions, educate patients, and collaborate with an interdisciplinary team to deliver comprehensive, value-based care.
The role emphasizes compassionate care for at-risk patients, ongoing education, and adherence to state regulations.
Qualifications
- Master’s degree in nursing required.
- Board Certification as an Adult, Adult-Geriatric or Family NP required.
- Current State RN & NP licensure required.
- BLS certification required.
- 6 months NP experience in Primary Care/Internal Medicine.
- DEA registration or eligibility.
- Experience providing care in patients’ homes preferred.
Responsibilities
- Develop and implement clinical plans of care for adult patients with chronic conditions.
- Perform history, exam, and diagnostics in clinic, telehealth, or home setting.
- Create and adjust treatment plans aligned with patient goals.
- Collaborate with interdisciplinary team to meet physical and psychosocial needs.
- Provide care for complex patients and families with compassion.
- Educate patients on medications, nutrition, and crisis prevention.
- Coordinate care with physicians and Upperline clinical team.
- Maintain accurate medical records per regulations and policy.
- Participate in weekly team meetings and continuing education.
