Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in the Augusta, GA Clinic, developing complex care plans for adults in the clinic, patient’s home, or via telehealth.

In collaboration with the local physician team, APPs educate patients, coordinate with the interdisciplinary team, and ensure care aligns with patient goals and values. This role emphasizes compassionate, detail-oriented care within a rapidly growing value-based care environment.

Qualifications

• Master’s degree in nursing is required.
• Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner is required.
• Current State RN & NP licenses are required.
• DEA registration or eligibility is required.
• 6 months NP experience in Primary Care/Internal Medicine preferred.
• Experience with value-based care is preferred.

Responsibilities

• Develop and implement clinical plans of care for adults with chronic conditions.
• Gather patient history, physical exam, and diagnostics in clinic, telehealth, or home.
• Develop and implement treatment plans aligned with patient goals.
• Collaborate with interdisciplinary team for physical and psychosocial needs.
• Prescribe medication as permitted by the State Board of Nursing.
• Maintain patient medical records and documentation per regulations.
• Participate in weekly interdisciplinary team meetings and ongoing education.
