Job summary

Your Health Organization in Augusta, GA is seeking a Nurse Practitioner to provide in‑home patient care across a designated service area. As a Provider, you will evaluate high‑risk patients, develop care plans, and coordinate with an interdisciplinary team to improve outcomes.

This is a full‑time, salary‑based position with monthly bonus potential and a comprehensive benefits package including a company vehicle, retirement plans, generous PTO, and HDHP options.

Qualifications

• NP license in good standing for the state of employment.
• Experience in home health, clinic, facility, telehealth, or specialty care preferred.
• Strong communication and collaboration within an interdisciplinary team.
• Ability to evaluate high‑risk patients and coordinate care plans.
• Willingness to travel between offices and care sites.

Responsibilities

• Evaluate high‑risk patients and develop individualized care plans.
• Coordinate care activities with interdisciplinary team members.
• Communicate regularly with case managers, coordinators, and other staff.
• Participate in team meetings to discuss patient progress and plans.
• Document care activities accurately for compliance and care management.
• Provide patient education and resources for self‑management and prevention.
