Job summary

Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in Kennestone/East Cobb, GA. You will develop plans of care, educate patients, and collaborate with an interdisciplinary team for comprehensive, value-based care. A weekday schedule and opportunities for growth are highlighted throughout the role.

Ideal candidates have NP certification, BLS, and experience in primary care or home-based settings, with a focus on compassionate, patient-centered care.

Qualifications

• Master’s degree in nursing required.
• Board Certification as an Adult, Adult‑Geriatric or Family Nurse Practitioner required.
• BLS certification required.
• 6 Months NP experience in Primary Care/Internal Medicine.
• Value-based care experience preferred.
• DEA registration or eligibility.
• Comfort providing care in patients' homes.
• Experience supervising clinicians preferred.
• Must have a reliable automobile with license and insurance; mileage reimbursed.
• Experience with EMR AthenaHealth preferred.
• Excellent oral and written communication with patients and team.

Responsibilities

• Develop and implement clinical plans of care for adult patients with chronic conditions.
• Gather patient history, physical exam, and diagnostics in clinic, telehealth, or home visits.
• Develop and implement treatment plans aligned with patient goals and conditions.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Provide compassionate care for complex patients and their families.
• Educate patients and families about medications, diet, and crisis prevention.
• Coordinate care with community providers and Upperline clinicians.
• Review patient records and care plans with local physician per policy.
• Maintain medical records per state regulations.
• Participate in weekly interdisciplinary meetings and education sessions.
• Provide telephonic support for escalated triage calls.
• Engage in continuing education per state requirements.
• Prescribe medications as permitted by state licensing.
• Other related duties as necessary.
