As an Advanced Practice Provider, you'll play an integral role in our care teams by providing high-quality, cost-effective care to patients.

We are seeking a Nurse Practitioner or Physician Assistant to:

• Provide patient-centered care floating to a variety of primary care clinics within UW Health including Family Medicine and General Internal Medicine, and be part of the clinical team at the employer-based UW Health Sub-Zero Clinic.
• Perform annual physical exams and help keep patients up to date on health maintenance.
• Care for the acute and chronic needs of patients from newborn to elderly.

Education:

• If Nurse Practitioner: Master’s or Doctorate’s degree from an accredited Nursing Program.
• If Physician Assistant: Bachelor’s or Master’s degree from an accredited PA Program.

Work Experience:

Minimum – Two (2) years of relevant clinical experience

Licenses and Certifications:

• Minimum – State of Wisconsin NP or PA licensure
• If Nurse Practitioner:
• Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND
• Active board certification as an NP in area of clinical practice. These may include the following: Family.

• If Physician Assistant:
• Active PA license, including prescriptive authority, in state(s) of practice AND
• Active PA board certification
