UW Health is seeking a Nurse Practitioner in Family Practice for a position in COTTAGE GROVE, Wisconsin.
This is a full-time, dayshift position, Monday through Friday. Hours may vary based on department needs. The work location is the Cottage Grove Clinic.
As an Advanced Practice Provider, you'll play an integral role in our inpatient and outpatient care teams by providing high-quality, cost-effective care to patients. We are seeking a Nurse Practitioner or Physician Assistant to:
Education:
Nurse Practitioner: Master’s or Doctorate from an accredited Nursing Program.
Physician Assistant: Bachelor’s or Master’s from an accredited PA Program.
Licenses and Certifications:
Minimum – State of Wisconsin NP or PA licensure.
Nurse Practitioner: Active RN and APRN certifications, including prescriptive authority, and active board certification in family practice.
Physician Assistant: Active PA license, including prescriptive authority, and active board certification.
All applicants must have or be able to obtain an active DEA license.
