American Physiatry is seeking an experienced Nurse Practitioner to oversee clinical operations within its growing post-acute care organization. This remote role supports multiple post-acute care facilities and requires strong SNF knowledge, process-building, and cross-functional collaboration.

  • Strong understanding of SNF operations, interdisciplinary care, and regulatory requirements.
  • Excellent organizational, communication, and leadership skills.
  • Comfortable working in a fast-paced, growing organization.
  • Experience with EMRs and healthcare technology platforms preferred.
  • Ability to manage multiple priorities while maintaining attention to detail.

  • Collaborate with facility leadership, nursing staff, and healthcare providers to improve workflows and operational performance.
  • Assist in the development, implementation, and refinement of clinical and operational processes.
  • Monitor key operational metrics and identify opportunities for improvement.
  • Help coordinate transitions of care and ongoing patient management initiatives.
  • Provide clinical guidance and support to internal teams as needed.
  • Participate in training, education, and quality improvement efforts.
  • Ensure compliance with applicable regulatory and documentation standards.
  • Serve as a liaison between internal teams and facility partners to facilitate effective communication and problem-solving.

Job description

We are seeking an experienced Nurse Practitioner with a strong background in skilled nursing facilities (SNFs) to help oversee and support clinical operations within our growing post-acute care organization. This is a remote position supporting clinical and operational initiatives across multiple facilities. This role is ideal for someone who understands the day-to-day workflow of nursing homes, enjoys building processes, collaborating with interdisciplinary teams, and improving operational efficiency across multiple facilities.
